報告一例五十歲男性罹患左側睪丸附近之平滑肌肉瘤。臨床上，病人有一左側陰囊內無痛性腫瘤多年，最近幾個月以來，發現腫瘤大小突然變大，陰囊超音波顯示為一睪丸附近一具有雜亂回音性的腫瘤，骨盆腔電腦斷層及核磁共振檢查更確定此腫瘤，且認為是一睪丸附近的肉瘤，乃接受根治性睪丸切坐手術治療；經病理確定為高度惡性的精索平滑肌肉瘤。六個月後，追蹤顯示無局部復發或遠處轉移。因此，我們對此少見的病例提出討論，並作文獻回顧。

A 50-year-old male with paratesticular leiomyosarcoma was reported. Clinically, the pa-tient presented with a palpable, painless mass in the left scrotum for several years. Rapid in-crease in size of the mass was noted by the patient n recent months. Ultrasound for scrotum re-vealed a paratesticular mass with heterogeneous echogenicity. CT scan and MRI for the pelvis confirmed the lesion and suggested the possibility of paratesticular sarcoma. Radical orchiec-tomy with early clamping of spermatic cord was performed. The final pathological report was high-graded leiomyosarcoma of spermatic cord. After six months, the follow-up revealed no evi-dence of local recurrence or distant metastasis. We presented ths rare case of paratesticular leiomyosarcoma and reviewed the relevant literature. (J Urol R.O.C., 8:152-156,1997)
