圖章戒指細胞癌是一種十分罕見的原發性膀胱腺癌，其組織學上的特徵是細胞內含有豐富黏液素，細胞核被壓迫在細胞邊緣，形成類似圖章戒指的形狀；在其臨床進程則是快速且致命的，具有擴散性侵犯及早期轉移現象。本文發表一位七十歲女性病人因尿滯留求診，經超音波掃描、尿路攝影術及電腦斷層檢查，顯示膀胱基底部有一類似前列腺突起形狀的腫瘤。經施行膀胱、子宮、尿道根治切除術，病理報告顯示一非臍尿管的、純粹的、侵犯性的圖章戒指腺細胞癌，而且淋巴結有轉移。病人接受輔助性化學治療後出院，追蹤半年無復發。

Signet ring cell carcinoma is a rare variant of primary adenocarcinoma of the urinary bladder. Its histologic feature is a high mucin content in the cytoplasma; this compresses the nuclei to one side and brings about signet-ring appearance. The clinical course is fatally fulminant with diffuse invasion and early metastasis. This report concerns a 70-year old female who complained of difficulty in urination and with urinary retention. The image studies, including ultrasound, IVU (intravenous urography) and CT (computerized tomography) scan, showed a tumor mass over the bladder base eith the appearance of indenting prostate. Radical surgery (anterior exenteration) followed by adjuvant chemotherapy was performed. The final pathologic diagnosis was non-urachal, pure, invasive signet wing cell carcinoma of bladder with sgage P4N2MO. The patient remained well during a six-month follow-up. The literature is reviewed.
