前列腺特異抗原(PSA)是目前用來診斷前列腺癌症相當具有參考價值的標記之一,其正常範圍定在0.0~4.0 ng/ml。不過,許多因素如良性前列腺肥大及年齡老化等都可能會影響血清中PSA值的數值。進而影響其診斷的價值。本研究希望了解正常健康男性其血清PSA值的分佈情形,並以分佈圖的95%為上限,定出隨著年齡差異而變化的參考範圍。根據我們對542例年齡在40~79歲間健康男性的研究,血清PSA值隨年齡的變化有相當大的差異性,在40~49歲組其平均值是0.8 ng/ml,但在70~79歲組則是2.7 ng/ml,如果根據其分佈圖來計算,則40~49歲組的參考範圍是0.0~3.3 ng/ml,60~69歲組是0.0~5.1 ng/ml,60~69歲組是0.0~5.1 ng/ml,而70~79歲則是0.0~7.1 ng/ml。我們需要更多正常人的數值,來訂定一個更精確不同年齡層的正常範圍,使這個癌症標記在臨床上的準確性更高。

To understand the distribution of serum prostate-specific antigen (PSA) in aged men and its relationship to age, a prospective study was conducted in community-dwelling men age 40 to 79 years from December 1993 to august 1994. Totally 553 men participated in this study. Of those, 542 had no evidence of prostate cancer as determined by the combined analysis of digital rectal examination (DRE), transrectal ultrasound (TRUS) and serum PSA. These 542 men were eligible for data analysis. The mean age of this study group was 56.7±8.4 years. The mean PSA value was 1.4±1.7 ng/ml (range 0.0-14.3) which increased from 0.8±0.9 ng/ml (range 0.0-4.1) in men aged 40 to 49 years and to 2.7±2.8 ng/ml (range 0.0-10.9) in those aged 70 to 79 years. Twenty-six men (4.8%) had a PSA value of greater than 4 ng/ml and less or equal to 10 ng/ml, and 3 (0.6%) had a PSA level of greater than 10 ng/ml. Serum PSA level showed a modest correlation with age (r=0.30, P<0.0001). By using the 95th percentile as a cutoff point, an age-specific reference range of serum PSA was created. It is 0.0-2.3 ng/ml for men aged 40 to 49 years, 0.0-3.3 ng/ml for men aged 50 to 59 years, 0.0-5.1 ng/ml for men aged 60 to 69 years, and 0.0-7.1 ng/ml for those aged 70 to 79 years. Using these data, 27 men (5.0%) would have had a PSA level above the reference range. It was concluded from this study serum PSA level is affected by age to some extent, and therefore this factor should be considered when using PSA as a marker for the diagnosis of prostate cancer. In addition, more data should be obtained from normal individuals to establish an age-specific range, and this may increase the sensitivity and specificity of the PSA test.
