為了探討Rigiscan在診斷男性性功能失調之角色,我們對135位男性患陽萎的病人進行了此項研究。病人的年齡從46歲到76歲(平均64.2),所有病人至少接受了兩晚的Rigiscan檢查,同時也接受了陰莖都卜勒超音波檢查,注射藥物後陰莖海棉體攝影檢查及生物振動覺儀檢查以評估病人的血管及神經方面的狀況。我們以Rigiscan的檢查結果和其他的檢查作比較並分析它們之間的相關性。其中有16位(69.5%)同時檢查出有靜脈滲漏(venous leakage);在其餘112位Rigiscan不正常的病人中有52位(46.4%)有靜脈滲漏,1位(0.9%)有動脈血流不足,59位(57.2%)同時有動脈及靜脈的異常,17位(115.2%)同時有神經方面的異常。總之,這些Rigiscan不正常的病人至少有一種或以上的器官性異常引起陽萎,且正常的Rigiscan也並不能代表病人有正常的血管或神經的狀況;所以我們的結論是不能夠單獨以Rigiscan來做心因性或器官興男性性功能失調之鑑別診斷,尤其是當Rigiscan顯示正常時。

英文摘要

To Better understand the role of Rigiscan in the evaluation of impotence, thorough examinations were conducted in 135 impotent patients in the age range from 47 to 76 years (mean 64.2). All patients underwent at least a two night Rigiscan examination, and also received penile Doppler ultrasonography, pharmacocavernosography and biothesiometry examinations to evaluate vascular and neurological status. The results of Rigiscan were compared with other examinations to clarify correlation among them. In this study, Rigiscan exhibited as normal in 23 patients, though 16 of them (69.5%) had venous leakage. In the 112 patients with abnormal Rigiscan, venous leakage accurred in 52 patients (46.4%), arterial insufficiency in 1 (0.9%), and venous leakage coexisting with arterial insufficiency in 59 patients (52.7%). Neurological deficit was found in 17 of these 112 patients (15.2%). All patient with abnormal Rigiscan findings had at least one underlying organic cause. Normal Rigiscan monitoring results do not usually imply a normal vascular status. The conclusion was that Rigiscan monitoring for the evaluation of impotence is not so accurate and essential as previously thought. Accordingly, differential diagnosis of psychogenic or organic impotence should not be made based only on Rigiscan examination, especially when the result shows normal.
