我們報告一例罕見的主動脈剝離是以左側肢體無力為表現的案例,一位57 歲男性凌 晨因左側偏癱、左側顏面神經麻痺與口齒不清,至急診就醫,腦部電腦斷層未發現顱內出 血而以缺血性腦中風的臆斷住院,住院後頸部血管超音波發現左頸總動脈剝離,懷疑主動 脈剝離,同日的胸腹部電腦斷層證實升主動脈與降主動脈皆有剝離,即刻轉診醫學中心治 療,病人不幸於轉診第二天死亡。在此提醒臨床醫師左側偏癱的缺血性腦中風可能由主動 脈剝離造成。
We report a rare case of aortic dissection presenting as left hemiplegia. A 57 year-old man visited the emergency department with left hemiplegia, left facial palsy and slurred speech. Brain computed tomography showed no hemorrhage. This patient was admitted under the impression of ischemic stroke. Neck sonography the following day revealed dissection of the left common carotid artery. Aortic dissection was highly suspected. Chest and abdominal computed tomography confirmed this diagnosis. This patient was referred to a medical center for further treatment, and died two days later. We remind physicians that aortic dissection could be the cause of left hemiplegia.
