中文摘要

目標:現今,三度空間模擬手術及電腦輔助設計與製作手術導板爲正顎手術中不可或缺之一環。然而,鮮少作者討論此技術運用於須施行前部根尖下截骨術之患者。因此,此篇文章討論以EXOCAD軟體執行三度空間模擬手術及電腦輔助設計製造導板之手術成效。

材料與方法:於高雄醫學大學附設醫院口腔顎面外科執行之10例上顎前部根尖下截骨術及7例下顎前部根尖下截骨術,共10位患者。將三度空間掃描之齒模及錐狀斷層掃描之數位檔案輸入軟體(EX0CAD)後,執行電腦輔助手術模擬,並取得「手術預計」之前顎骨塊移動量。「實際執行」之前顎骨塊移動里則由術後之雜狀電腦斷層掃描檔案上取得。此兩組數據相互比較,取得手術精準度之相關資料。

結果:所有手術皆順利執行。手術預計及實際執行移動量之差距,皆小於1.5毫米。P値顯示皆大於0.05,此表示手術預計之移動量及實際執行之移動量間並無顯著統計差異。藉由以上數値,可得知由EX0CAD軟體執行之手術模擬及導板製作,有可接受之精準度。

結論:三度空間手術模擬及電腦輔助設計與製作手術導板在手術成效上有高精準度,可嘉惠於日後之手術患者。

英文摘要

Objective: Nowadays, three-dimensional (3D) virtual surgical planning and computer-aided design/manufacturing (CAD/CAM) splints are necessary elements in orthognathic surgery. This study investigated the surgical outcomes of virtual surgical planning and palatal splints, and lingual splints manufactured by EXOCAD 2016.10 (Exocad GmbH, Darmstadt, Germany).

Materials and Methods: Ten maxillary and seven mandibular anterior subapical osteotomy (ASO) were performed in a total of 10 patients in the Oral and Maxillofacial Department of the Kaohsiung Medical University Hospital. After importing the 3D data of digital dental casts and cone-beam computed tomography (CBCT), a virtual surgical planning was performed in EXOCAD. Movement amount of segmented anterior jaw indicated the "surgical plan." Data of "actually performed" bone segment movement were collected in post-operation CBCT. We compared these data to analyze the accurate results.

Results: All the operations were performed smoothly. The average difference between surgical planning and the actual surgical outcome was within 1.5 mm. The P-value showed that all distances between preoperative planning and postoperative outcomes were more than 0.05, which expressed no statistical difference. Based on these data, surgical guides fabricated from the EXOCAD software were acceptably precise.

Conclusions: The 3D virtual surgical planning and CAD/CAM fabricated surgical splints indicated high accuracy in postoperative facial outcomes. This would be highly beneficial in the future.
