背景:1995年間,臺灣發生了食用減肥菜 (Sauropus androgynus) 而導致嚴重肺功能異常的事件。本研究以鎝-99m-DTPA放射霧氣肺造影、高解析度肺部電腦斷層攝影比較肺功能檢查的結果,以探討食用減肥菜而導致肺功能障礙。
方法:本研究包含10位原來健康,但因食用減肥菜而導致肺功能障礙的女性病人,並比較她們的鎝-99m-DTPA放射霧氣肺造影、高解析度肺部電腦斷層攝影及肺功能檢查的結果。
結果:其中9位病患有異常的鎝-99m-DTPA放射霧氣肺造影結果:包括了不均勻的放射性霧氣肺分佈9位,放射性霧氣由肺部清除速率增加8位。肺功能檢查則顯示了9位病患皆有阻塞性肺病而1位合併有侷限性肺病,但是10位病患中只有3位的高解析度肺部電腦斷層攝影結果異常。
結論:本初步研究結果顯示鎝-99m-DTPA放射霧氣肺造影比高解析度肺部電腦斷層攝影對於檢驗食用減肥菜造成的肺部損傷來的靈敏。
Background: In 1995, there was an outbreak of Sauropus androgynus (SA) associated pulmonary dysfunction in Taiwan. This study investigated lung injuries resulted from consuming SA relying on a combination of 99mTc-DTPA radioaerosol inhalation lung scintigraphy (DTPA lung scan), PFT, and HRCT and compared their diagnostic utilities.
Methods: Ten patients who had been previously healthy and developed respiratory distress after consuming the SA vegetable were enrolled in our study to evaluate the utilities of technetium-99m-DTPA radioaerosol inhalation lung scintigraphies (DTPA lung scan), high resolution computed tomography (HRCT) and pulmonary function tests (PFT) for diagnosis of their conditions.
Results: Nine of the patients had abnormalities on their DTPA lung scans, including inhomogenous radioaerosol deposition (n = 9), faster clearance of radioaerosols from the lung (n =8), and both (n= 8). In PFT, the same nine patients had abnormal findings, including obstructive type (n = 9), restrictive type (n = 1), and both (n =1). However, there were only three HRCT results considered to be abnormal among these 10 patients.
Conclusion: Our preliminary results indicated that DTPA lung scan was more sensitive than HRCT. suggesting its usefulness as an additional tool in the diagnosis and follow-up of lung injuries induced by SA.
