背景:本研究之目的在於評估紅斑性狼瘡腎炎病患,其實驗室數據、鎵-67造影與腎臟穿刺結果:如急性活性指數activity index (AI)、慢性活性指數chronicity index(CI)及World Health Organization(WHO)分類之間的關係。
方法:27位紅斑性狼瘡腎炎病患(平均年齡31.9±11.3歲),接受腎臟穿刺、鎵-67造影、anti-double-strained DNA(antidsDNA)、血清肌酸酐(serum creatinine)和24小時尿蛋白等檢查。
結果:我們的數據顯示:(1)鎵-67造影和AI的相關性最好,24小時尿蛋白和AI也有很好的相關性;(2)血清肌酸酐是唯一和CI有相關性的檢查;(3)所有檢查和WHO分類相關性均不好。
結論:本研究建議鎵-67造影和24小時尿蛋白可以用來評估紅斑性狼瘡腎炎活性,而血清肌酸酐則可評估預後。

英文摘要

Background: The aim of this study was to evaluate the relationship of various laboratory data, and gallium image, to changes in renal biopsy measures of activity index (AI), chronic index (CI) and World Health Organization (WHO) classification in lupus nephritis.
Methods: Twenty-seven patients (mean age 31.9±11.3 yrs.) with lupus nephritis underwent renal biopsy procedures, gallium scan and laboratory examinations including anti-double-strained DNA (anti-dsDNA), serum creatinine and 24-hour urine protein.
Results: Our data revealed: (1)Gallium scan shows the best correlation with AI and 24-hour urine protein also shows good correlation with AI. (2)Serum creatinine is the only parameter which shows good correlation with CI. (3)All parameters show poor correlation with WHO class.
Conclusion: We suggest that gallium scan or 24-hour urine protein may be a useful method in the evaluation of disease activity, while serum creatinine may be useful in the prediction of disease outcome.
