鎝－99m-MIBI副甲狀腺造影的高敏感度與精確度使許多罹患副甲狀腺機能元進的病人在進行手術前會轉介到核醫科做術前評估。此外，對於再度複發的病人也可以用此檢查來定位。文獻報告指出，甲狀腺結節腫大、甲狀腺腺瘤、與甲狀腺癌都會造成鎝－99m-MIBI副甲狀腺造影呈現攝取增加而導致可能的錯誤判讀。然而在此次的病例中，我們討論一個可能引起錯誤判讀的情況；這是一位六十歲的女性病患，甲狀腺切除手術後的血液中甲狀腺激素濃度低下，促甲狀腺激素(TSH)分泌增加導致甲狀腺錐狀葉(Pyramidal lobe)增生，鎝－99m-MIBI副甲狀腺造影則呈現明顯攝取增加，而不容易和副甲狀腺腺瘤做鑑別診斷。

The 99mTc-MIBI parathyroid imaging is useful for evaluation of hyperparathyroidism. False-positive findings have been reported in cases with thyroid nodular goiter, adenoma, and carcinoma. However, functioning thyroid remnant, especially stimulated by high TSH level, has not been mentioned. The uptake in hyperplastic pyramidal lobe deserves special attention in patients after thyroid surgery.
