通過醫學檢驗室認證?提升檢驗(查)品質之重要指標,也是檢驗室提昇品質所追求的目標與未來之趨勢。目前國外已有影像醫學相關之認證系統,例如美國的北美放射線學院以及紐西蘭的核子醫學實驗認證聯合委員會等,其中在管理要求上和目前之ISO15189類似,但在技術要求上則略有差異,不過對於整體上要求檢查前、檢查中及檢查後之品質的精神是一致的。本文主要依據ISO15189之精神並參考國外文獻及國內目前作業規範,歸納出一套核醫影像檢查品質之技術指引,其中包括,(1)組織與架構(人員、環境與設施、儀器與設備及檢查作業服務量等);(2)核醫作業程序(臨床作業程序、受檢者管理作業程序、設備品管作業程序及報告與紀錄作業程序等)及(3)品質評估及改善(品質評估、滿意度調查、品質改善討論會及聯合討論會等)。我們初步建立一套核醫影像檢查品質要求的技術指引,希望可以提供國內各核醫影像檢查室參考,以建立適用於國內核醫科的品質規範,並不斷地追求高品質的服務。

英文摘要

Medical laboratory accreditation is the cornerstone for quality control, thus, an accredited laboratory represents the good quality. Although there are no available accreditation program regarding nuclear medicine imaging study in Taiwan currently, but in other countries such as the American College of Radiology (ACR) and the Intersocietal Commission for the Accreditation of Nuclear Medicine Laboratories (ICANL) are already providing those programs. In this article, we would like to depict the essential criteria for nuclear medicine imaging study technical guidelines based on ISO15189 and alien specifications. There are three major components: (1) structure and organization: personnel, accommodation and physical facilities, instrumentation and equipment, volume of clinical procedures; (2) process of nuclear medicine procedures: clinical procedures protocol, patient management protocol, equipment quality control protocols, reporting and record protocols; (3) quality assessment and improvement: quality assessment, satisfaction investigation, quality improvement meeting and combined conference. It could be the fundamental for the preparing of nuclear medicine laboratory accreditation in the future.
