Early identification and precise localization of the causes of fever of unknown origin(FUO) are important for guiding further diagnostic procedures and subsequent treatments. A 57-year-old male patient visited our hospital due to intermittent low grad fever, dry cough, and generalized ache for several weeks. Physical examination and a series of examinations were negative findings. Conventional anatomic imaging modalities showed no significant abnormality whereas Tc-99m MDP bone scan and Ga-67 scan revealed some inconclusive abnormalities. In contrast, F-18 FDG PET scan demonstrated diffusely increased radiotracer uptake in the bone marrows and splen and contributed to the final diagnosis of acute lymphoid leukemia (ALL).This case further confirms that F-18 FDG,a "catch-all" radiotracer, is of great value in evaluating patients with FUO because it can accumulate in infections, inflammatory diseases, and malignancies, which are the three major causes of FUO.
