Brain metastases now account for about one half of all brain tumors. The discovery of the primary lesion is clinically important, as their presence can affect the therapeutic approach decisively. We reported two cases with brain metastases of unknown primary origin who underwent PET/CT and were found to have lung cancer. A 45-year-old female patient presented with right side weakness. Recent CT and MRI studies showed multiple metastastic lesions in the brain. Chest radiograph showed no lung lesion. PET/CT showed a area of increased uptake in left upper lung. Biopsy proved lung cancer. Another 74-year-old female patient presented with left side weakness. A recent MRI study showed multiple metastastic lesions in the brain. Chest radiograph showed no lung lesion. PET/CT showed a area of increased uptake in left lower lung and high suspect lung cancer.
