Endovascular aneurysm repair (EVAR) is a new technology which recently becoming the common procedure for the treatment of abdominal aortic aneurysm or thoracic aortic aneurysm. Although EVAR is apparently less invasive as compared to major open surgery, some complications, i.e. post-implantation syndrome, stent infection, migration, endoleak, and fracture, still cannot be avoided. This present report describes a febrile case after endovascular stent repair with persistent stent inflammation. Unexpectedly a non-tuberculous mycobacterial (NTM) infection was regarded as the prime diagnosis. We discuss the diagnostic and therapeutic challenges which were encountered and review the current literature on this evolving subject. The role of Ga- 67 scans is in initial screening and therapeutic monitoring by serial follow-up.
