篇名 The stability study of factor structure in Toddlers with ADHD

Objective: Recent studies suggest that Attention Deficit/Hyperactivity Disorder (ADHD) is a neurodevelopmental abnormality which is associated with gene. Therefore, a systematic research about early identification and intervention during infancy is worthwhile. Researches in last 15 years using scales to assess behavioral problem type and severity of ADHD find that there are two major factors: inattention factor and hyperactivity/impulsivity factor. This study is focused on whether the factor structure is stable from infancy to childhood. Method: This study was carried out from 2003 to 2011 in south of Taiwan and participated by mothers with 1.5-to 3-year-olds toddlers. 477 mothers as informants included 403 mothers in normal community group and 74 mothers in premature group. All mothers had completed Disruptive Behavior Rating Scale-toddler(DBRS-toddler) modified from DBRS-parent (Huang & Chao, 2006) and Home Situation Questionnaire(HSQ, Barkley & Murphy, 2001). All mothers had signed informed consent. Result:The result about factor structure of DBRS-toddler suggested that the fitness of three factor model (inattention factor, hyperactivity/impulsivity factor and oppositional/defiant factor) was better than the fitness of two factor model which combines inattention factor and hyperactivity/impulsivity factor into one factor. However, fitness index of these two models was very good. As a result, study method of using HSQ to differentiate two models (Erford, 1998) was applied. If three factors model was correct that inattention factor and hyperactivity/impulsivity factor were two different factors, the correlations between HSQ and these two factors should be distinct. On the contrary, it was hard to differentiate the correlations between HSQ and these two factors in two factor model. Consequently, three factor model and two factor model were distinguished from each other. The result suggested that manifestation of three factor model was still better than two factor model and similar to the results of three factor model of DBRS in childhood and preschool. Conclusion: Disruptive behavior has universally stable factor structure and the factor structure is present stably in different ages (DuPaul, 1998 ; Friedman-Weieneth, 2009), different scales (Swanson, Nolan, and Pelham, 2006) and different cultures(McMenamy , 2011). For this reason, it is important to discuss stabilities from different aspects.
