中文摘要

     本篇個案報告是敘述一位接受「活體捐腎手術」母親的護理經驗。以Gordon十一項健康功能為評估指引,發現個案雖對於捐出腎臟的意願堅定,但仍有自責、怨老天不公平的情緒,且不了解腎臟手術方式及預後,有較不切實際的期待。在接受捐腎手術後,本身遭受手術後急性疼痛,且擔心兒子腎移植手術的預後,呈現出焦慮、知識缺失等問題。在護理過程中筆者運用護理指導、傾聽、陪伴、引導其說出心中感受及情緒之抒發、並配合醫師給予說明捐腎手術的合併症及照護、提供適宜的疼痛處理及引導家屬提供心理支持等,增進個案對捐腎及腎移植的了解,減輕其焦慮與疼痛情形。現今探討有關接受活體捐贈者的照護文章並不多,期望此篇報告之內容能做為相關照護計劃之參考。

英文摘要

     This case report describes the experience of the nursing care of a patient undergoing surgery to make a living kidney donation. The Gordon 11-item Functional Health Pattern was used as an assessment tool to assess the client's health problems. Although having the confidence to donate a renal organ, the client had several emotional frustrations, including self-blame and guilt. After surgery, the patient's major problems were pain, anxiety, and knowledge deficit. In order to increase the understanding of the renal transplantation, alleviate the level of anxiety and pain suffered, we taught the patient and her family related knowledge, listened to and accompanied the patient, provided appropriate pain management, and encouraged family members to provide mental support. There was little literature discussing the care of living kidney donors. We hope this report can serve as a reference for planning the nursing care of living donors.
