     本研究旨在建立並測試「尿毒症病患訊息需求量表」,研究分二階段進行。階段一是發展研究工具(instrument development):研究者查證與訊息需求及透析治療相關文獻,並與腎臟專科醫師、護理師及透析病人討論,再加上本身的臨床經驗來發展問卷項目(item),並由八位專家審閱確立問卷的內容效度。階段二是測試量表的信效度:本研究以內在一致性信度、二週間隔再測信度及因素分析來檢定尿毒症病患訊息需求量表。本研究採問卷訪談法,立意取樣南部22家設有透析中心的醫院或診所之144位尿毒症病人為研究對象。「尿毒症病患訊息需求量表」經由因素分析萃取出五個因素,分別為:了解腎臟移植的可行性、認識透析治療、瞭解透析治療對個人、家庭及社會層面的影響、認識尿毒症、瞭解飲食及水份的控制。此五個因素可解釋的總變異量為49.36%,顯示本量表具有良好的建構效度。在信度方面,「尿毒症病患訊息需求量表」之五個因素的Cronbach's a再測信度係數分別介於0.80-0.94與0.70-0.82,顯示本量表具有良好的內在一致性且具穩定性。

英文摘要

     The purpose of this study was to develop and to test an instrument for measuring informational needs of uremic patients undergoing dialysis treatment. The study was divided into two phases, developing the instrument and testing validity and reliability. In phase one, the researcher reviewed the literature about informational needs and dialysis treatment, discussed with nephrology physicians, HD clinical nurse specialists and uremic patients and wrote down personal experiences serving as sources for item derivation. Eight experts reviewed the instrument for content validity. In phase two, the psychometric properties of the Informational Needs Scale in Uremic Patients were examined: including internal consistency reliability, two weeks test-retest reliability, and the dimensionality of the Informational Needs Scale in Uremic Patients through factor analysis. A structural interview method was employed in the study. A convenience sample for this study consisted of 144 uremic patients who received dialysis treatment in the dialysis center of twenty-two hospitals in Southern Taiwan. Five factors within the Informational Needs Scale in Uremic Patients were identified by factor analysis, including (1) understanding the feasibility of kidney transplantation, (2) knowledge of dialysis treatment, (3) understanding the influences of dialysis treatment on personal, family and social life, (4) understanding uremia, and (5) understanding dietary and fluid control. These five factors explained 49.36% of variance of the Informational Needs Scale in Uremic Patients. The coefficient of the Cronbach's a and test-retest reliability in these five factors of the Informational Needs Scale in Uremic Patients were 0.80-0.94 and 0.70-0.82, respectively. The results showed that the instrument had a satisfactory internal validity and reliability.
