對原發性副甲狀腺亢進的病人而言,鎝-99m-MIBI掃描已是目前用來偵測及定位副甲狀腺腺瘤的重要檢查。本文報告一例位於左側下頷下區域的異位性副甲狀腺腺瘤的罕見病例,我們成功地以鎝-99m-MIBI偵測並加以定位。在前位的影像上,這個病灶的表現並不明顯,但加上斜位的影像,則可清楚的分辨出來。緊接在攝影之後,配合術中加馬探討的導引,這個副甲腺腺瘤快速及安全地被切除下來。
99mTc-MIBI scanning has become the preferred mode to detect and localize parathyroid adenomas in patients with primary hyperparathyroidism. Here we report and rare case of ectopic parathyroid adenoma located in the left submandibular area. The lesion was successfully detected and localized by the 99mTc-MIBI parathyroid scan. It was unremarkable in the anterior image of the neck, but was clearly demonstrated on the oblique view. Subsequently, this parathyroid adenoma was resected rapidly and safely with the guide of a intraoperative gamma probe.
