前言:本研究之目的在於比較使用鎝-99m-HMPAO單光子電腦斷層攝影('9mTc-HMPAOSPECT)、電腦斷層(CT)、磁採 這影(MRI)及腦電波國(EEG)來偵測Creutzfeldt-Jakob病(cm)之腦病變。
方法:14位CJD病換接受鎝-99m-HMPAO SPECT ' CT ' MRI及EEG來偵測大腦皮質及基底核並比較其結果。
結果:鎝-99m-HMPAOSPECT顯示所有14位cm病忠於大腦皮質及基底核皆有多發、廣泛且不規則腦血流減少現象。 8位病患之CT或MRI僅顯示不明顯之腦萎縮現象。14位病車之EEG皆有異常之腦電波(PLEDs)於兩側大腦半球。
結論.本研究結果顯示若與CT或MRI比較,鎝-99m-HMPAOSPECT是一種偵測CJD病換腦病變比較靈敏的工具。
Background: Our purpose was to detect brain lesions in Creutzfeldt-Jakob disease (CJD) using brain technetium-99m hexamethyl-propyleneamineoxime (99mTc_ HMPAO) imaging in comparison with computed tomography (CT), magnetic resonance imaging (MRI) and electroencephalography (EEG).
Methods: 99mTc-HMPAO brain images, using fan-beam single-photon emission computed tomography (SPECT), were used to detect cerebral cortex and basal ganglion anomalies in CJD. Fourteen CJD patients received brain 99mTc-HMPAO imaging, and CT. MRI and EEG were also performed for comparison. Results: 99mTc-HMPAO brain images demonstrated multiple diffuse and irregular hypoperfusion areas over the cerebral cortices and basal ganglia in all 14 patients. In 8 of 14 cases, brain CT or MRI showed only unremarkable aged brain atrophy. EEG revealed periodic lateralizing epileptiform discharges (PLEDs) over the bilateral hemispheres in all 14 patients.
Conclusion: This study suggests that, in comparison with brain CT or MRI, 99mc-HMPAO brain imaging is a more sensitive tool for the detection of brain anomalies in CJD patients.
