在急診如遇到意識不清、骨折疼痛或酒醉病患，由於病人無法配合等因素，易造成影像上模糊或不清楚，且病患頸部或患肢嚴重骨折時，移動傷處可能會造成二次傷害的發生。因此本研究中，主要探討急診外傷病患如果是套有頸圈及明顯骨折者，以採取不移動病患為原則，而利用移動病床、擺放片子位置與X光球管投射角度等方法，分析是否可以得到與單純轉動患肢之相同的診斷結果。由結果中發現，病患只要維持平躺的姿勢，利用X光球管與片匣之間的相對關係下，不但診斷骨折的價值與單純轉動患肢相同外，還可增加病患檢查之安全性，及降低二次傷害之發生。

When encountering drunk, unconscious or severe traumatized patient with agony pain, poor patient cooperation results in poor image quality. Secondary injury is also possible when transferring the traumatized patient with severe fracture of C-spine injury. In this article, we discuss the alternative method of examination for these patients while keeping the patient on the transfer tank and meticulous change the position of the X-ray tube and cassette. The goal is to achieve equal diagnostic yield as the traditional way by transferring patient to the radiological examination table. We find that by just meticulous change the position of the X-ray tube and cassette, we can keep the patient lying supine on the transfer tank and get the equal diagnostic yield as traditional method. Patient stability and safety are secured, and secondary injury is avoided by such alternative method.
