慢性腎臟衰竭是一個重要的疾病。近幾年來，不論在台灣或是其他國家，透析人口的發生率及盛行率都大幅上升。慢性腎臟衰竭會產生高血磷、活性維他命D缺乏及次發性副甲狀腺亢進症；進一步將引起腎性骨病變、全身性骨質減少及骨折。在環境毒物中，很多有機化合物以及重金屬，可能會導致腎臟病變與腎衰竭。許多勞動工作者有慢性腎臟衰竭，且需長期從事一些粗重工作，因此是腎性骨病變及骨折的高危險群。本文將回顧腎性骨病變及口服磷結合劑的相關文獻，以期能讓讀者了解腎性骨病變及口服磷結合劑的重要性。

Chronic kidney disease is an important disease. The incidence and prevalence of dialysis have increase progressively over the past year in Taiwan and whole the world. Chronic kidney disease complicates hyperphosphatemia, hypocalcemia, low serum levels of vitamin D and secondary hyperparathyroidism. Furthermore, renal osteodystrophy, osteoporosis and bone fracture happen easily. Many labor workers have the history of chronic kidney disease and always need to do heavy work for a long time. This population is high risk for renal osteodystrophy and bone fracture. In this article, we review the disease, discuss the rationale for treatment with oral phosphate binders and suggest an approach for clinical practice.
