Secondary testicular malignancies are very rare. The incidence is about 0.02%~2.5% from autopsy studies. Most metastasize from leukemia and lymphomas, followed by prostate and lung cancers. To our knowledge, testicular metastasis from esophageal cancer has never been reported. We report a 51-year-old man diagnosed with esophageal cancer who presented with a right testicular lump. An ultrasound scan suggested a malignant mass. A radical orchiectomy was performed, and the pathologic examination revealed squamous cell carcinoma, suggesting metastasis from esophageal cancer. Although testicular metastasis is rare, it is prudent to check the scrotum during a physical examination of patients with cancer.
