A nulliparous woman with history of sigmoid carcinomahad metastatic Kruk.enberg tumors noted at her antenatal care at 16 weeks' gestation. Since the patient wished to continue with her pregnancy, a debulking surgery including bilateral oophorectomy was performed to spare a room for the fetal development. Chemotherapy was postponed until one month after childbirth. A subsequent Tc-99m MD Phone scans displayed omental caking and metastatic bone lesions. Our patient presented with bilateral Krukenberg tumors, peritoneal carcinomatosis and bone metastasis simultaneously, which hinted at synchronous and different routes of tumor spreading. The omental cake showed diphosphonate uptake, a pattern that to our knowledge has rarely been described before, is also discussed.
