中文摘要

醫療服務缺乏整合協調,效益不佳,造成健康照護提供者與接受者之滿意度低落,多數醫療相關的異常事件, 根本原因在醫療團隊內部缺乏合作與溝通。跨領域團隊合作照護教育(Interprofessional Education, IPE),其目的在 於培育出「具備團隊合作照護能力的醫療專業人員」來達到跨領域團隊合作照護(Interprofessional Practice, IPP), 最終目標是希望提升全人照護醫療品質。IPE 指發生在兩個以上專業領域間的教學活動,各醫療專業人員共同學 習的過程中,透過溝通、協調及整合,以促進專業間的合作。理想的IPE 教案架構,內容包括核心(core)與情境 特定(contextualized)的知識、態度及技能等六部分。以病人為中心,訓練共通性的團隊能力,學習恰當應用合作 照護所需的知識、態度及技能等核心能力,且此三面向須擴展至特定情境中。在該案此類情境下,所可提供的醫 療服務專業知識,釐清病人真正所需的醫療照護,各專業人員針對此類病人擬定周全延續性的照護計畫。本教案 案例探討頭頸部癌病人,於本院所接受之醫療照護,分析各職類醫護人員於醫療照護行為中所扮演之角色,透過 跨領域團隊合作照護教育, 讓不同職類之醫療照護團隊成員更瞭解彼此之業務特性與專業, 掌 握團隊合作的知能與技巧,並建立各職類間良好的溝通管道,以提升醫療照護品質與增進病人安全。

英文摘要

Clinical performance lack integration is causing low patient satisfaction. Insufficient communication between medical departments leads to many complex medical issues. The purpose of Interprofessional Education (also known as "IPE”) is to educate students on how to enhance their ability toward Interprofessional Practice (also known as "IPP") as a step to our final goal in an improved healthcare outcome for patients. IPE occurs in two or multi-professional education activity, each profession shares a common education procedure through communication, teamwork, and integration to gain a well-balanced interprofessional collaboration. An ideal IPE education structure combines core and contextualized knowledge, attitude, and skill. By implementing collaborative teamwork knowledge, attitude, and skill to build a patient based situation. In those circumstances, individual professions have to provide the most adequate healthcare that can sustain the life-long treatment plan. In this report, we are primarily focused on nasopharyngeal cancer patients that have resided in our hospital for multi-professional healthcare. The goal is to analyze each role that the individual profession performs in the multi-team professional collaboration in order to share common knowledge and enhance interaction for an improved healthcare quality.
