本文探討外科部醫師手術排班問題,建構一周期時間內分派醫師進行手術 之決策模式。在台灣,許多醫院面臨病床缺乏或各科別間病床分配不均而導致 病患延遲手術的情形。本研究之多目標排班模式其主目標為最小化空病床數, 次目標包括優先排入大手術與優先排入醫師喜愛的時段。研究中提出兩階段的 方法進行求解。第一階段針對單一科別醫師手術排班問題,第二階段則針對外 科部門科別與科別之間病床調度問題,最後獲得改善後之空病床數以及排班結 果。本研究求解醫師數50,病床數為200 時,第一階段與第二階段的總求解 時間在20 秒以內,顯示本研究所提出的方法是有效率的。

英文摘要

The problem of master surgery schedule involves the decision making on allocating physicians’ surgeries to the defined time shifts. It is prevalent that some elective patients are blocked because of lacking of bed space and the bed imbalance among departments is quite common in many hospitals in Taiwan. The main objective of this paper is to minimize the number of empty beds, accompanied with subsidiary objectives including heavy surgeries prioritized and physicians’ preferred shifts. We developed a two-stage approach to solve the problem. In the first stage each individual department aims to independently find its master surgery schedule, and in the second stage, a borrowing measure among departments is carried out providing the measure is permitted. We show that the approach can solve problems having 50 physician and 200 beds for each department within 20 seconds.
