本研究是以修訂美國Rush Medicus Tool---Monitoring the Quality of Nursing Care量表為基礎，建立本土化的腫瘤護理品質標準，並發展護理品質過程評值量表，進而依此量表進行臨床腫瘤病房之護理品質評值性研究。研究時間自民間82年8月1日起至民國83年7月31日為止。研究對象是以本院38個內外科病房所有腫瘤住院病人，以隨機取樣10%為調查對象，實訪210人，先作病人分類後再取問卷(含四大類，十九份問卷)施以評值。問卷之信度採觀察員間信度( inter-rater reliability )，觀察員的一致性為90%，觀察員內在信度( intra-rater reliability )達100%一致性，效度採專家效度法。
研究結果顯示一般腫瘤住院病人的護理品質分數在目標一「擬定護理計畫」平均得分為87.11分，目標二「確保病人生理需要」得分為95.68分，目標三「確保病人心理社會文化靈性需要」得分為96.14分，目標四「執行護理目標之評值」得分為90.89分。比較不同分類腫瘤住院病人在同一目標的護理品質得分以變異數分析及 Scheffe' test 結果顯示:四類病人在同一目標，如目標一、二、四的護理品質得分均無顯著差異，但在目標三上有顯著差異。另比較同一類腫瘤住院病人在不同四大目標，如目標一、二、三、四的護理品質得分則有顯著差異。本研究結果可提供護理行政者、護理教師及護理人員做為改善腫瘤科護理服務品質之參考。

The purpose of this study was to establish a set of oncological nursing care standards, develop an evaluation tool, and monitor the quality of nursing care. The survey of the quality of nursing care for oncological patients started on August 1, 1993 and ended on July 31, 1994. All medical and surgical wards from 38 units were chosen as test units and a total of 210 oncological patients were monitored. Tests for inter-rater and intra-rater reliability were used. There was 90% agreement between 2 raters and 100 % agreement in observer herself for intra-rater reliability. The quality scores for Objective I「formulating nursing care plan」 was 87.11; Objective II 「attending patient's physical needs」 was 95.68; Objective III 「attending patient's psycho-social-spiritual-culture needs」 was 96.14; and Objective IV 「the achievement of objectives」 was 90.89. There was no significant difference in the quality of nursing care for four types of patients on the same objective scores except Objective 3. There were signifi-cant differences on the quality of nursing care scores for Objective 1,2,3 and 4 for four types of patients. The re-sults will help nurses, nursing administrators, and educators know the strengths and weakneSses of nursing care and help to improve the quality of nursing care.
