護理壓力為護理專業成長的重要環節,護理人員與工作環境互動過程中所產生之壓力可為負面的身心症狀,也可激發出高度之生產力,本研究目的即在暸解台灣醫學中心護理人員之壓力,並建立測量護理人員壓力表之信效度,以項目內在一致性查驗信度,以內容、專家、及建構效度查驗效度。護理人員壓力量表經專家譯為中文,初步並與13位具中英文能力之護理人員會談,確定該量表內容之易讀性與適用性。本研究選擇有2200名護理人員之台灣某醫學中心,隨機抽取569位,回收511份資料,以SAS套裝軟體作統計分析。研究結果發現台灣護坦人員壓力之Cronbach's α值為0.93,顯示該份量表在信度方面,具有良好之內在一致性。再以因素分析方法測量該量表之結構效度,共可萃出四個因素,分別為個人反應、工作關注、勝任以及無法完成私人工作等四個因素,可解釋之總變異量為80.1%,顯示台灣護理人員壓力量表頗能反應台灣護理人員之壓力。而由四個因素之內容來看,涵蓋了護理人員與環境互動的結果,包括對醫療環境評估、個人專業能力之期許,以及面對兩者衝突之身心反應。此一研究結果有助臨床護理人員及護理行政人員對工作壓力之暸解,為進一步發展護理專業或設計護理人員壓力處置方案之基礎。

英文摘要

Understanding nurses' stress is vital in the nursing profession because nurse stress is closedly related to the growth of the nursing profession. The purpose of this study is to build up a reliable and valid instrument to measure nurse stress in Taiwan. A Nurse Stress Checklist was selected and translated into the Chinese language. Then, 569 nurses were systematic randomly chosen from a Medical Center with 2200 nurses. Questionnaires were distributed to these nurses from a clerk system. 511 questionnaires were returned and data were analyzed by SAS software for personal computer. Results showed that Cronbach's a was 0.93, demonstrating a high level of internal consistency. Factor analysis was performed to find the structure of the Taiwan Nurse Stress Checklist. Four factors were extracted and named, including personal responses, work concerns, competency, and incompleteness of personal arrangement. Four factors accounted for 80.1% of the total variance, showing that the results of the Taiwan Nurse Stress Checklist closedly reflected the nurses' work stress. The content of these four factors embraced the process of nurses interacting with their environment, such as evaluation of the health delivery system, expectation of one's professional competency, and the psychosocial responses to the dilemma between the above situations. The results of this study provide a reliable and valid instrument for nurses and nursing administrators to understand nurse stress in Taiwan, which is a basis for designing stress management for nurses and further professional development.
