本研究目的是探討失能老人遷居至安療養機構之心路歷程及其相關因素，採茁根理論研究法，以深度訪談為主要收集資料方式。訪談過程以錄音記錄，再逐字抄錄為書面訪談過程，資料以持續比較分析法及個案內與個案間比較來分析整理。理論性選樣共10位居住在台北市已(未)立案安療養機構中的失能老人參與研究，並請其中抽象思考能力較佳的4位老人做參與者檢定。結果發展出一個循環、動態的A承命生根之過程B模式，共包含四個過程要素：覺察現存情境、量測差異、產生情緒反應和採取承命生根策略。而另有五項前置情況會影響老人在A承命生根之過程B中對相同情境的認知及感受改變的程度、影響其對遷居的接受度、和對生活變化所做的調適意願有不同的影響。此發現除可瞭解台灣老人遷居的心路歷程及心理感受，並可對臨床護理措施提出本土性的建議，而所發展的模式架構可供未來進一步實質理論發展的參考。

The purpose of this study was to explore the process of frail elders'relocation to institutions in Taiwan. Grounded theory method was used in this study.A theoretical sample of ten frail elders was obtained from four nursing homes.In-depth interviews, participant observations and member checks were used to collectdata. Interview data were subjected to constant comparative analysis both within andacross cases. Findings of this study suggest that accepting fate and taking root is theprocess used by frail elders to achieve or preserve wellness after relocating to thenursing home. The process of accepting fate and taking root includes four components:recognizing the real conditions, weighing the differences, triggering emotionalreactions, adopting accepting fate and taking root strategies. The results of this studycan increase nursing knowledge about elders' psychological process of relocation toinstitutions; therefore, effective nursing interventions can be developed. The tentativetheory provides a framework for further substantive theory development and relatedresearch conducts.
