中文摘要

目的:了解現今救護車公司運作的概況及車上照顧品質,並提出對民間社護車幸者的品質改善方案。
方法:從2002年1月至9月收集兩家救護車公司的轉院紀錄表,針對表格內的項目加以統計分析。
結果:在轉院原因及目的地方面,分別以緊急轉院41.4%及他院急診47.2%占最多,沒有附轉院摘要佔8.6%。分析轉送病患的時間與量測生命徵象的次數呈中度正相關,且其結果不因分層分析而改變。量測生命徵象的次數與病人呼吸頻率及意識有相關。
結論:救護車照護品質大體上是可以接受的,但未附轉院摘要及利用救護車載送遺體的現象需改進。應持續督導業者從事品質改善工作,導入評鑑手段加以分級管理,以保障轉診民眾之權益。

英文摘要

Objectives: To understand the present situation and to evaluate contents of on board medical care provided by private ambulances during their daily operation.
Methods: Retrospective analysis of inter-facility transfer sheets from two plivate ambulance companies in Tainan region since January 2002 to September 2002. Comparison of actions of medical care taken in the preceding hospital and subsequent transfening ambulance was performed.
Results: Over 41.4% transferals were emergency transport, 47.2% of transferals destinatlOn were hospital emergency department. There were 141 (8.6%) inter hospital transports without transfer note. Frequency of vital sign measurement on board is correlated with total transport time span, conscious level and respiratory rate.
Conclusions: In general, quality of medical care during inter-hospital transfer by private ambulances is acceptable, however, lack of transferal note provided by health care fadlities misuse in mortuary service need to be improved. Continuing quality monitoring by systemlc evaluation will be mandated to render best interest to the consumers.
