方法:為評估99mTc標幟抗乳腺癌粘蛋白單株抗體(McAb)放射免疫影像(RH),及比較99m Tc-McAb與99m Tc-MIBI乳腺影像詮斷乳腺癌及腋淋巴結轉移的臨床價值,本研完對17例乳腺瘤患者進行RII,其中9例間隔2-3天同時進行99m Tc-MIBI影像比較研究。
結果:9例原發展性乳腺癌中,8例為RII陽性,靈敏度88.9%;5例原發性乳腺癌伴隨腋淋巴結轉移病例中的4例,及1例右乳腺浸潤性導管癌根治術5年後出現右腋淋巴結轉移病例,RII發現淋巴結轉移;相反的,6例乳腺良性病變和1例右乳腺單純癌根治術7年後定期隨訪病人,RII為陰性。9例比較研完結果:5例原發性乳腺癌及其中的的2例伴腋淋巴結轉移病例,99m Tc-McAb RII和99m Tc-MIBI兩種影像均為陽性,但1例乳腺導管癌病例的同側1枚腋淋巴結反應增生,99m Tc-MIBI影像卻呈假陽性;其餘4例乳腺良性病例,RII陰性,其中1例99m Tc-MIBI 5分鐘時相影像圖呈假陽性。
結論:99m Tc標幟抗粘蛋白McAb和99m Tc-MIBI兩種影像均能靈敏地診斷乳腺癌及其淋巴結轉移,而99m Tc標幟抗粘蛋白McAb RIl特異性更強。

英文摘要

Background: The aim of this study was to evaluate the efficacy of radioimmunoimaging (RII) with 99mTc-anti-mucin McAb in detecting breast cancer and to compare the accuracy of 99m Tc-McAb and 99m Tc-MIBI in detecting primary breast cancer and lymph node metastases.
Methods: Seventeen patients with suspected breast lesions with/without axilla lymph node metastases were included in this study. All patients received intravenous injection of 740-925 MBq of 99m Tc-McAb. Nine of 17 patients received 740 MBq of 99m Tc-MIBI injection at the interval of 2 - 3 days after McAb injection for the comparison study of McAb and MIBI. At 2, 4, 5 and 24 hrs postinjection of McAb, or at 5 rain and 1.5 - 2 hrs postinjection of MIBI, planar lateral and anterior views with 1000 k to 2000 k counts were obtained. SPECT was performed in some of the studied cases.
Results: RII showed positive images in 8 of 9 primary cancer patients, with sensitivity of 88.9%. In the malignant group, 6 of the 11 had axillary lymph node metastases. Localization of McAb in the axillary node metastases was observed in 5 of them. The sensitivity was 83.33%. Six patients with benign breast lesions and 1 patient with post-operative breast cancer without recurrence showed negative findings (TN) on McAb scan. For the comparison study of 9 patients, all of 5 known primary breast lesions and 2 axilla node metastases in these 5 patients were seen by both imaging procedures. MIBI gave false positive image of one reactive proliferation lesion in a patient with ductal cancer. Four benign lesions on McAb and 3 of these 4 Pesions on MIBI images were negative. One false positive lesion was seen on 5 min MIBI image.
Conclusion: Both McAb and MIBI were sensitive in breast cancer imaging. McAb was more specific in detecting primary breast cancer and lymph node metastases.
