目的:研究鉈-201腫瘤攝影於口腔癌之應用。
方法:23位診斷為口腔癌的病人接受鉈-201腫瘤掃描檢查。不同的掃描技術,包括全身掃描(whole body scan)、平面攝影(focal views)以及單光子激發電腦斷層攝影(SPECT)均加以取像(腫瘤大小以及病理學上的相關性亦被記錄及分析。
結果:鉈-201腫瘤攝影對於偵測口腔也之敏感度,以不同的掃描技術攝影,有不同的結果。平面損影以及單光子射出電腦斷層攝影的敏感度分別為65.2%以及82.6%(口腔腫瘤小於2公分時,掃描對腫瘤偵測率不佳(但若特腫瘤大小與細胞分化程度和鉈-201腫瘤攝影偵測率相較,發現腫瘤大小與細胞分化程度並非決定腫瘤攝取鉈-20101之絕對因素。
結論:鉈-201腫瘤攝影有助於區分口腔癌的惡性分化程度,但口腔癌病灶大小不是決定鉈-201攝取的主因。對於局部淋巴轉移病灶,鉈-201的偵測率不佳。相關的研究工作可能可以借助增加注射鉈-201的劑量來加以改善。

英文摘要

Background: The purpose of this study was to determine the clinical usefulness of thalrium-201 tumor scan for detecting oral cancer.
Methods: Twenty-three patients with oral cancer were studied by thallium-201 tumor scan. In addition to whole body scan, focal views and single photon emission computed tomography (SPECT) were also performed. Tumor uptake with clinical information and pathological findings of head and neck region were correlated and analyzed.
Results: The detection rates of focal views and SPECT were 65.2% and 82.6%, respectively. Tumor detection rate was decreased in scanning small tumors (less than 2 cm in diameter). However, the degree of cancer cell differentiation and tumor size were not the determinative factors of tumor uptake of 201TI in oral cancer.
Conclusion: 201TI tumor scan seems to be useful in differentiating law grade from high grade oral cancer. However, the tumor size is not the determinative factor for the accumulation of 201TI in tumor cells. 201TI is also not useful for the detection of regional lymph node metastasis. Further research is needed, such as to increase the detectability of 201TI by increasing the injection dose. Moreover, 201TI SPECT can increase the sensitivity of tumor detection in oral cancer. In technical aspect, whole body scan can not offer more information for detection of distant metastasis than focal views.
