背景:臨床作業中常發覺到,雖然是採用相同的分析方法,但在使用不同的採血試管時,部分的檢驗項目結果會有不同之影響。本研究之目的即在確認此差別之存在。方法:本研究同時以玻璃(glass BD Vacutainer(下標 TM) tube)及塑膠(Plastic BD Vacutainer SST(上標 TM) tube)材質之採血試管收集臨床970位自願者之血液檢體,以放射免疫分析法分析全三碘甲狀腺素(total triiodothyronine; TT3)與全四碘甲狀腺素(total thyroxine; TT4)。結果:以玻璃採血試管所測之TT3及TT4(113.7ng/dI; 8.5μg/dL)分析值比塑膠採血試管(110.3ng/dL; 8.39μg/dL)高,P值分別爲<0.0001及0.0013,具統計之意義。再進行Passing-Bablok回歸分析結果顯示TT3與TT4分析值在二種試管間無明顯之差異,但TT4則呈現負向比例性差異。討論:在進行臨床分析檢驗之前,實驗室應了解或評估不同採血管影響檢驗分析之結果,以提昇檢驗品質與醫療品質,並避免由此所產生之分析結果差異。

英文摘要

Background: Accurate laboratory studies are essential for accurate diagnosis. However, several factors may affect test results. This study sought to identify the impacts of two different collecting tubes on clinical analysis results. Methods: Glass and plastic collection tubes were used for blood collection in 970 subjects. Two thyroid related hormones, total triiodothyronine (TT3) and total thyroxine (TT4), were examined by means of radioimmunoassay. Blood samples were collected into the tubes simultaneously and randomly. Results: Test values were significantly higher among specimens in glass BD Vacutainer(superscript TM) tube than the plastic BD Vacutainer SST(superscript TM) tube (TT3: 113.7ng/dL vs. 110.3ng/dL; P<0.0001 and TT4: 8.5µg/dL vs. 8.39µg/dL; P=0.0013). Regression analysis by Passing-Boblok method revealed there is no definite difference in the TT3. However, negative proportional difference was found in TT4 analysis. Conclusion: The collection tube can influence analytic outcomes. Medical laboratories should pay more attention to variations in results caused by different collecting tubes.
