一離心機在運轉時,離心機轉鼓及外殼突然破裂,造成一人罹難死亡。致災之離心機轉鼓及相關組件經拉伸試驗、斷口巨觀、微觀觀察及強度計算等綜合分析後,研判致災因素可能為轉鼓銲道因入料不足,導致銲道與轉鼓銲接處產生間隙,造成該處應力集中及間隙腐蝕,致使整條銲道遭裂縫貫穿,最後轉鼓加強箍因無法承受運轉負載而斷裂,導致整個離心機轉鼓及外殼向外彈出而致災。分析結果建議轉鼓桶身銲接處應避免打洞,以免銲接處因孔洞而造成應力集中,進而誘發裂縫生長,另轉鼓桶身銲接時銲料應完全入料,以維持其應有強度。

英文摘要

A fatal accident was caused by the drum burst of a centrifuge on October 25, 2005. Based on mechanical tests, macro-and micro-fractographs, and stress analysis, we conclude that insufficient welds and punched holes along the weld are most likely to be responsible for the accident. The insufficient welds resulted in stress concentrations and local corrosion, and the local corrosion cavities then condensed together over time. Finally, the stiffeners yielded and fractured due to overloading during drum operation. The drum burst damaged the outer shell of the centrifuge and collapsed the whole system in the process. The investigation suggested that the weld should not be punched in order to avoid stress concentration and that welding practice is necessary in order to guarantee the strength of the welds.
