我們報告一個臨床上診斷是皮質基底核退化症之病例。一個另性表況有運動不能僵硬症、肢體肌肉張力不全、肌躍症、皮質感覺失常、智力衰退等症狀。腦部核磁共振顯示大腦右側皮質萎縮,尤其是頂葉。鎝-99m-ECDSPECT檢查 發現右側大腦,除視覺皮質以外,腦灌流不足。氟-18-FDGPET檢查呈現兩側瀰漫性代謝下降,特別是右側,包括大 腦皮質、視丘及基底核。鎝-99m-TRODAT-I SPECT攝影指出在右側紋狀體活性低下,而且尾狀核及被殼呈現同等程度 下降.
We report a case of probable corticobasal degeneration (CBO) diagnosed by clinical criteria. A 43-year-old male developed unilateral akinetic-rigidity syndrome, limb dystonia, stimulation-sensitive myoclonus, cortical sensory impairment, and intelligence decline. Brain magnetic resonance image (MRI) showed cortical atrophy on the right side, especially parietal lobe. Technetium- 99m-ethylenedicysteine diethylester ("9mTc-ECO) cerebral perfusion single photon emission computed tomography (SPECT) demonstrated hypoperfusion in the right hemisphere with visual cortex sparing. 18F-fluorodeoxyglucose ('8F-FOG) positron emission tomography (PET) revealed diffuse hypometabolism in bilateral hemisphere, thalamus, and basal ganglion, more severe on the right. 99mTc-TROOA下1 SPECT disclosed hypoactivity in the right striatum with equal involvement in both caudate nucleus and putamen.
