中文摘要

皮膚疾病是門診中常見的問題,雖然大多數的皮膚疾病不會致命,但對病人的工作
及生活影響卻很深遠。在所有職業疾病中,皮膚疾病是除了外傷和重複性傷害外最常見
的問題。根據美國勞工統計局(Bureau of Labor Statistics)資料顯示,職業性皮膚疾病占所有職業性疾病約10-15%,以2000年的統計資料顯示年發生率為46/100,000。其中約有
20-25%的勞工因職業性皮膚疾病而無法工作,平均天數為9-11天,造成的經濟損失為
2.2-10億美金,當中包含每年醫療支出和勞工補償費用。所以職業病醫師在發現疑似職
業性皮膚疾病時,需詳細詢問病人病史,工作性質及工作環境中有無暴露於可能致病的
物質。在治療同時也能提供勞工及企業主職場安全衛教,讓勞工不但能把疾病治療,也
減少職業性皮膚疾病的再發生率,同時降低醫療成本支出及維持職場勞動力。本文提供
一位因化學物質引起之職業性皮膚炎病例,並回顧相關文獻,提供給職業病醫師診斷及
處理職業性皮膚炎的方法,改善勞工工作品質。

英文摘要

Skin disease is one of the most common problems in clinics. Although most of the skin disease can not be fatal, it is very far-reaching but influences the working and living of the patient.
In all occupational diseases, skin disease is the most common problems except trauma and repeated injury. Bureau of Labor Statistics data show that occupational skin diseases accounted for approximate 10% to 15% of all cases of occupational illnesses and the annual incidence
was 46/100,000 in 2000. It has been reported that 20% to 25% of workers with occupational skin disease lost time from work, an average of 9 to 11 days. The total annual cost of occupational skin diseases may range from $222 million to $1 billion including annual medical costs and worker’s compensation. As a result, the occupational physicians need to explore patient's medical history, working properties and environment whether exposed to the sensitizer material in detail while clinical features are suspected occupational skin disease. We can offer treatment and occupational safety education to worker and employer at the same time. It can let workers not only to cure their disease and decreased the recurrent rate of occupational skin disease but also to reduce the medical costs and maintain workforce at the same time. We describe
a case of occupational skin disease caused by chemical material and review relevant literature to the occupational physicians about the methods of diagnosis and management the occupation skin disease and how to improve the quality of work.
