盲腸腺癌在臨床上較少被報導，一般在手術前也很少被懷疑。我們報告一例23歲女性因右下腹痛及右側腰部不適，來本院泌尿科求診，經靜脈腎盂攝影檢查發現膀胱右側有被壓迫的痕跡且右側之腎臟及輸尿管均無法顯影出來。因擔心顯影劑造成腎臟之傷害故安排腹部核磁共振檢查，此時除了右側腎水腫及右側輸尿管擴大外，亦發現在右側卵巢有多囊性之腫瘤造成膀胱右側及右側下輸尿管之壓迫及在盲腸附近另外有一腫瘤。之後安排手術，包括右側大腸切除、右側卵巢腫瘤切除 (左側卵巢因生育問題而保留) 及淋巴腺摘除。術後，病理檢查發現盲腸腺癌合併卵巢及淋巴腺轉移，病人於住院第11天出院且之後接受化學治療。

Primary adenocarcinoma of the appendix is an uncommon disease, especially the colonic type, rarely suspected before surgery. We report a 23-year-old female of colonic-type adenocarcinoma of the appendix with right ovarian metastasis which compressed the lower third portion of right ureter resulting in right hydronephrosis and right hydroureter, presenting with right lower quadrant pain and right flank discomfort.
Right hemicolectomy, right oophorectomy and lymph nodes dissection was performed. The patient was discharged on the 11th hospital day. Subsequently she received chemotherapy.
