背 景 代謝症候群個案常有尿酸上升的情形,高尿酸血症可能與代謝症候群的惡化有關。
目 的 比較有無代謝症候群學生的尿酸濃度差異,及探究高尿酸血症與代謝症候群之相關。
方 法 採橫斷式研究設計,以1,096位大學新生為研究對象,資料收集包括個案基本屬性、身體質量指數、腰圍、血壓、血糖及血脂。代謝症候群及高尿酸血症分別以國民健康局、台灣地區營養及健康調查報告為依據。
結 果 高尿酸血症及代謝症候群盛行率為16.97%及8.10%,男性均高於女性。在調整干擾變項後,代謝症候群個案之尿酸濃度,顯著高於無代謝症候群之個案,且尿酸濃度與代謝症候群異常項目呈顯著上升趨勢。高尿酸血症之個案在代謝症候群有較高風險;在代謝症候群項目分析中,男性高尿酸血症者比起正常尿酸者,有較高腹部肥胖和高血壓的風險,女性則有較高腹部肥胖、高血壓和高密度脂蛋白膽固醇過低的風險。
結 論/實務應用
高尿酸血症可引起某些健康問題,與代謝症候群有正相關。學校衛生護士應利用教育資源,發展理想的介入措施,有效協助學生改變生活型態,降低代謝症候群及高尿酸血症的發生。

英文摘要

Background: Serum uric acid (SUA) level is often elevated in participants with metabolic syndrome (MS), and hyperurice-mia appears linked inextricably to the presence of MS.
Purpose: Aims of this study included conducting a comparison of SUA concentrations in university students with and without MS and exploring the relationship between hyperuricemia and MS.
Methods: A cross-sectional study was conducted on 1,096 freshmen students, who received a health examination at a university. Data were collected on demographics, body mass index, waist circumference, blood pressure, fasting plasma glu-cose, and lipid profile. Criteria used in this study for MS and hyperuricemia were those published by the Bureau of Health
Promotion, Department of Health, Taiwan, ROC, and in Taiwan’s Nutritional and Health Survey.
Results: The prevalence of hyperuricemia and MS in all par-ticipants was 16.97% and 8.10%, respectively. The prevalence of both was significantly higher in male than in female par-ticipants. Covariance analysis showed that SUA levels were significantly higher in participants with MS than in those with-out, after adjusting for confounding variables. The components
of MS increased significantly with increased SUA levels. Par-ticipants with hyperuricemia faced a higher risk of MS than did those with normal SUA levels. In terms of MS analysis com-ponents,male participantswith hyperuricemia faced higher risks of abdominal obesity and hypertension than did their peers with normal SUA concentrations, and female participants with hyperuricemia faced higher risks of abdominal obesity, hyper-tension, and low high-density lipoprotein cholesterol.Conclusions and Implications for Practice: Hyperuricemia may trigger certain health problems and has been positively related withMS. To improve these concurrent conditions, school nurses should make use of education resources to develop optimal intervention strategies to facilitate effective lifestyle changes in students and reduce the incidence of MS and hyperudicemia.
