背景 懷孕因生理結構及荷爾蒙變化,可能造成孕婦身體活動或睡眠品質改變而影響其生產方式。目的 探討身體活動、睡眠品質與非計畫性剖腹產之關係。方法 運用前瞻及相關性之研究設計,立意取樣台北巿兩家醫學中心的第三孕期婦女共184位個案。以三日身體活動量表(3-day physical activity)及匹茲堡睡眠品質量表(PSQI)評量其身體活動之能量消耗與睡眠品質。結果 顯示研究對象的身體活動量較一般族群低,亦較少從事中重度的身體活動。睡眠品質差者佔77.2%,有40.7%的孕婦對自己的睡眠品質感到不滿意,而造成睡眠品質差的主要面向為「睡眠困擾」。Logistic迴歸分析顯示睡眠品質變差,發生非計畫性剖腹生產的機率增加為1.22倍(95% CI=1.01-1.48),孕期三日身體活動量增加則是減少為0.76倍(95% CI=0.60-0.97)。結論/實務應用 建議產前照護應主動探詢孕期身體活動及睡眠品質的狀況,並給予適當協助與衛教,有助於減少非計畫性剖腹產的發生。

英文摘要

Background: Physiological structure and hormonal changes in women during pregnancy may alter physical activity behavior and sleep quality, both of which are factors that influence the method of fetal delivery. Purpose: The purpose of this research was to examine the relationships between physical activity, sleep quality, and unplanned cesarean section. Methods: This study employed a prospective and descriptive correlational design. A total of 184 pregnant women in their third trimester at 2 medical centers participated in the study. A 3-day physical activity scale and Pittsburgh Sleep Quality Index (PSQI) were used to evaluate, respectively, physical activity-related energy expenditures and sleep quality. Results: Subjects had a comparatively low level of energy expenditure on physical activity and performed a lower than normal amount of moderate-to-heavy physical activity. Three-quarters (77.2%) of subjects had poor sleep quality, and 40.7% reported being unsatisfied with their sleep quality. The main cause of poor sleep quality was sleep disturbance. Using logistic regression, results indicated that the odds of undergoing cesarean section increased to 1.22 (95% CI=1.01-1.48) when sleep quality was "poor". By contrast, odds decreased to 0.76 (95% CI=0.60-0.97) when physical activity energy expenditures rose. Conclusions/Implications for practice: Results support the need to obtain physical activity and sleep quality data on women during pregnancy. Appropriate assistance and health education are important during perinatal care to reduce the incidence of unplanned cesarean sections.
