背景 疲憊是乳癌病患在接受化學治療時常見及困擾的副作用,實有必要一介入性措施來減輕疲憊。目的 本研究旨在測試自我調節方案對初次接受化學治療乳癌病患因應疲憊之成效。方法 本研究採雙組後測之類實驗研究設計。研究工具之結構式問卷內容包括個人基本資料問卷、症狀困擾量表、情緒困擾次量表及台灣版簡明疲憊量表。實驗組於第一次至第二次化學治療期間,共計進行四次活動,每次約20分鐘之自我調節方案;對照組則接受一般的常規照護,隨後兩組於第二次及第三次化療當日,治療前一小時分別執行後測一及後測二。結果 研究結果顯示自我調節方案能有效提升實驗組病患因應症狀困擾以及情緒困擾之能力,降低症狀及情緒困擾,並有效緩解疲憊。結論/實務應用 本研究結果可供未來臨床發展化療疲憊照護的參考依據,並可將本研究所發展的自我調節方案納入初次化療病患照護流程的一部分。
Background: Fatigue is a common and disturbing side effect faced by breast cancer patients who undergo chemotherapy that may be minimized through effective intervention. Purpose: This study was designed to examine the efficacy of a self-regulation protocol in helping breast cancer patients currently undergoing initial chemotherapy cope with fatigue. Methods: A quasi-experimental posttest-only with comparison group design was chosen for the study. Structured questionnaires employed included a participant demographic questionnaire, Symptom Distress Scale-Chinese Modified Form, Profile of Mood States, and Brief Fatigue Inventory-Taiwan Form. Patients in the experimental group were asked to follow a four session, 20-minute self-regulation protocol during a period that spanned their first and second chemotherapy cycles. Comparison group patients received routine health care institute nursing care. Two follow-up tests were administered, respectively, one hour prior to their second (post-test 1) and third (post-test 2) chemotherapy treatments. Results: Those in the experimental group improved significantly their ability to cope with chemotherapy-related distress symptoms and emotions, showed decreased symptoms and emotional distress, and significantly decreased levels of fatigue. Conclusions/Implications for practice: Findings may be used as a reference in developing chemotherapy-related fatigue care plans for cancer patients in the future. In addition, the self-regulation protocol presented in this study may be introduced as an integral part of nursing interventions in the future.
