上顎門齒因阻生而未萌出咬合面之患者，在矯正門診中時常可見，因上顎前牙對美觀的影響相當大，一旦換牙時間過久，父母與孩童皆易察覺而尋求治療。在此提出一例以矯正治療合併牙周手術方式，成功地將阻生之上顎中門齒牽引至咬合面並排列整齊的孩童患者以供參考。患者為十歲三個月的女童，因上報左側正中門齒尚未萌發，故經由牙科診所轉診至本院牙醫部要求治療。經檢查後發現，患者上報左側門牙區萌發空間不足，使得上報左側中門齒阻生，由於患者之父母相當在意美觀問題，故決定以矯正治療合併牙周手術方式，將阻生之上顎左側中門齒牽引至咬合面。經一年六個月完成矯正治療，阻生之左上正中門齒已拉出並與相鄰門齒排列整齊，由於患者仍有其他齒列問題，將來仍需進一步接受全口矯正治療。對於上顎門齒阻生的孩童患者，適時的矯正治療有助於改善其外觀，並增進孩童心理之健康。

It is usually to find the impacted maxillary incisors in the orthodontic clinic. For esthetic problem, parents and children came to the hospitals to seek the treatment for the unerupted maxillary teeth. A impacted maxillary central incisor case, which was treated by fixed appliance combined with periodontal surgery was presented in this report. This was a 10 year-old girl who was diagnosed as impaction of upper left central incisor and was referred from general dental clinic. After orthodontic examination and consultation with parents, she was treated by fixed appliance combined with periodontal surgery. The impacted maxillary left central incisor was traction to the occlusal plane after 18 months orthodontic treatment. The girl still needed the advanced orthodontic treatment in the future after the full mouth permanent teeth were erupted. For esthetic and psychological reasons, orthodontic treatment is needed for the children who have impacted upper incisors.
