本專案目的旨在降低內科加護病房的泌尿道感染率，專案進行期間為2007年11月至2008年11月，經由現況調查發現：（1）護理人員未依標準技術操作存留導尿技術及護理；（2）護理人員對於泌尿道感染控制及護理認知不足；（3）加護病房照護常規中未制定導尿管移除評估標準；（4）加護病房未制定導尿管照護標準作業規範；（5）加護病房內洗手設備的便利性不足；故於2008年3月1日起增訂加護病房「導尿管護理常規」之照護標準、制定移除導尿管的評估及取消尿壺共用之作業模式，舉辦在職教育課程及執行存留導尿技術護理評核與臨床設備改善。經由上述改善措施後，2008年6～12月平均感染密度下降至3.76‰，已達本專案的目的。期望經由本專案的經驗分享，提供其他單位的參考。

The purpose of this project is to reduce the urinary tract infectious rate in medical intensive care unit. This project started from Nov. 2007 to Nov.2008. Clinical investigation disclosed that urinary tract infection is related to 1. Urinary catheterization by nursing staff without follow the standard procedure 2. Inadequate knowledge about infection control and nursing skill 3. Absence of standard criteria for catheter removal 4. Absence of standard procedure about care of urinary catheter 5. Inconvenient equipment of hand-washing.
On 1 March 2008 several reforms were done including 1. Stipulation of stand procedure for nursing care of urinary catheter 2. Stipulation of the timing for catheter removal 3. Avoidance of sharing chamber pot 4. Education and evaluation for catheterization technique of nursing staff 5. Improvement of hand-washing equipment. After these reforms, the urinary tract infectious rate decrease to 3.76‰ during June to December 2008. We like to share the experience to other units.
