近年來臺灣的自殺率逐年攀高,且自殺病人出院甫出院期間是自殺的高危險期。因此,本研究的目的探討剛出院精神疾病自殺病人被家屬照顧的過程。採茁根理論(grounded theory)研究法,以訪談法對15位有自殺意念或企圖自殺的出院病人進行資料收集。資料的分析採用持續重複比較,並藉用QSRNUD*IST電腦軟體協助分析。研究結果發現「感受家屬自殺防範的照顧」爲最重要的核心類別。病人因爲「無法調適生活」而想自殺,然後被送入精神科病房接受治療與照顧。出院後他們返家被家屬的照顧,在整個家屬照顧過程中他們感受到「被日夜守護」如“被注意觀察”和“被注意安全”;亦感受到「被維持基本需求」如“被身體照顧”、“被送醫治療”和“被協助規則服藥”;另外亦感受到「被維持心理平靜」如“被關心”、“被寬容”和“被灌溉希望”。最後,六位病人會有「重獲生命的渴望」,但九位病人「再次有自殺意念和再次企圖行爲」。因此,家屬需要更多自殺防範的技能及尋求資源的協助。所以,精神科健康照護小組成員應主動衛教家屬有關自殺防範的技能及協助家屬建立支持系統,以減少自殺出院病人的自殺意念或企圖。

英文摘要

Recently, the prevalence of suicide in Taiwan has been increasing. Moreover, there is a particularly high risk of suicide following discharge from hospital. Thus, the purpose of this study was to investigate the process of psychiatric suicidal patients who have recently been discharged from hospital and have been cared for by family caregivers at home. A grounded theory approach was used. Fifteen patients who had either suicidal ideas or had attempted suicide were interviewed. Data was analyzed using the constant comparative method by the qualitative software program QSR NUD*IST. The core category emerged was "experienced suicide preventive care from family" Suicidal patients who felt an inability to cope with life, which led to suicide attempts, were then transferred to psychiatric wards for treatment and care. After discharge, they went home and were cared for by family. In the care process, they felt guarded day and night, as they were observed attentively and attention was paid to their safety. They also felt their basic needs were being maintained, as they were being provided physical care, being sent to hospital for treatment and taking medicine regularly. In addition, they also felt their mental tranquillity being maintained, as they felt being cared about, being accepted, and being instilled of hope. At the end, six patients regained the desire to live but nine patients still had suicide ideas and attempts. Therefore, family need more skills about suicide preventive strategies and resource finding. The mental health team should actively educate family about suicide preventive strategies and help them to build support systems in order to reduce patients? suicide ideas and attempts.
