多層切面電腦斷層掃描(Multi-slice Computer Tomography, MSCT) 為放射學影像設備與技術的最新進展。但產生之輻射劑量遠遠超過常規x光檢查,電腦斷層心臟冠狀動脈血管攝影(CT coronary angiography; CTCA)尤其為甚。綜觀相關研究報告指證輻射之負面結果,及國人罹患冠狀動脈心臟病之高死亡率,可能進而增加輻射風險。
遂進行本研究,以評估輻射有效劑量及潛在風險。本研究利用64層切面電腦斷層掃描儀(TOSHIBA Aquilion 64multi-slice CT, Japan),測量RANDO假體之心臟和危急組織器官所接受的吸收劑量。結果心臟有效劑量為0.850mSv,掃描範圍內鄰近之乳房組織為1.543 mSv,肝臟0.542 mSv,肺臟1.637 mSv,食道0.526 mSv,胃1.603 mSv;全身有效劑量為8.405 mSv。電腦斷層掃描若為合理的醫療需求,其相關風險相對於獲得的診斷信息其實是較小的。臨床科醫師應進一步了解電腦斷層掃描可能造成的潛在輻射傷害,審慎評估、權衡利弊,方可達成最可觀之效益。

英文摘要

Multi-slice computed tomography (MSCT), in the radiology imaging equipment and technology fields, with the latest progress. However, the radiation dose of MSCT produces far more than conventional x-rays, computed tomography coronary angiography (CTCA) in particular is even. Overview of research evidence related to adverse outcome of radiation,
and high risk of coronary heart disease (CHD) mortality in Taiwan, and this may increase the risk of radiation. Therefore, we conducted this study to assess the effective dose of radiation and the potential risks. In our study we used the protocol recommended for noninvasive visualization of the coronary arteries in our institution. This protocol involves the use of 64 multi-detector row CT (TOSHIBA Aquilion 64 multi-slice CT, Japan) to cover the RANDO phantom’s heart and adjacent critical organs. The accepted dose for the heart without cardiac gating based on ICRP 103 were estimated 0.850 mSv ;scanning range close to the breast tissue was 1.543 mSv; the liver was 0.542 mSv; lung was 1.637 mSv; esophagus was 0.526 mSv, the stomach was 1.603 mSv; the total body effective dose was 8.405 mSv. CTCA can achieve reasonable as long as the medical needs of its risks relative to the diagnostic information obtained is smaller. Clinicians should be aware of CT scan radiation can cause potential harm, and carefully assessed and weighed, can reach the most impressive benefits.
