中文摘要

周邊動脈血管阻塞(Peripheral Arterial Occlusive Disease, PAOD),會造成病患肢體缺血而產生間歇性的跛行,或者因缺血性疼痛而使肢體缺血性壞死增加。本實驗希望能運用數位減像血管攝影術(Digital Subtraction Angiography, DSA)針對PAOD患者的下肢血管情形,分析血管狹窄的嚴重程度並進行治療。對象是被臨床醫師判定為疑似PAOD患者,直接執行血管攝影檢查。使用Philips血管攝影儀器(型號:Allura Xper FD20/20),其注射條件分為兩段:第一段8 ml/sec,注射24 ml(3秒),第二段6 ml/sec,注射42 ml(7秒),共66 ml;攝影參數:Bolus Chase。其分析方法是利用血管定量法進行分析。本實驗共有30位患者進行檢查,其中有狹窄的患者共有19位。針對血管直徑、狹窄與口徑分析。結果得知有7位血管狹窄程度大於33%,則接受支架治療術。本實驗利用了血管定量法的分析技術,能夠協助醫師判定血管狹窄程度,選擇更適當的治療方式,以幫助患者減輕PAOD造成的不便。

英文摘要

Peripheral arterial occlusive disease (PAOD) results in limb ischemia and causes intermittent claudication. PAOD can also cause ischemic pain which leads to increased ischemic limb necrosis. The aim of this study was to use DSA (Digital subtraction angiography) to examine the vasculature of the lower extremity of patients with PAOD and to analyze the degree of vascular stenosis in order to provide treatment. The subjects of this study were patients who were diagnosed with PAOD. Accordingly, the patients were scheduled for an angiogram using Philips angiography equipment (Allura Xper FD20/20, philips, the Netherlands). The standard procedure for injection was as follows: 24 mL injection at 8 mL/sec and 42 mL injection at 6 mL/sec, for a total of 66 mL. Bolus chase was the imaging parameter and the analytical method was vascular quantification. Thirty patients were examined and 19 patients had stenosis. Through measurements of vessel diameter, stenosis, and vessel caliber analysis, seven patients were shown to have vascular stenosis > 33%. Accordingly, these patients underwent stenting. This study took advantage of vascular quantification analysis to assist physicians in determining the degree of vascular stenosis of patients with PAOD in order to select for more appropriate treatment methods and to relieve patients of their pain and physical disability.
