隨著時代變遷及醫學工程的進步,許多高科技的臨床醫療設備,不但快速發展且被廣泛的使用(如電腦斷層、超音波,核磁共振及X光機等),且拜電腦與通訊網路之賜,而發展出醫學影像儲存與傳輸系統PACS (picture Archiving and Communication System),藉以提昇醫院管理效率,降低人事成本及無片化的理想。本研究案採用TOSHIBA KXO-50G與TOSHIBA-15R一般診斷型X光機及Canon CXDI-11之DR系統之數位化接收板,本研究採三組分別爲PACS建置前與建置後,以及減少一名放射師,來做比較分析,每一名放射師每一小時之工作負荷量及病患候檢時間,研究顯示PACS建置前和建置後病患等待時間減少1/2以上,而每位放射師工作負荷無太大變化。而減少一名放射師後,每一小時每一名放射師平均工作負荷量增加約1.6人次,但病患平均候檢時間減少約20-68秒。本研究結果顯示,採用PACS與DR系統後,其人員工作效能增加,病患候檢時間縮短,對院方而言,同時達到管理效率及降低人事成本。對放射師而言,於人員調度,休假、專業技術訓練及影像品質控管更嚴謹,進而達到院方、病患與放射師三贏局面。

英文摘要

As the changes of time and the advancement of medical engineering, many high-tech medical clinical equipments which develop rapidly are widely available like CT, ultrasound, Mid, X-ray machine. Additionally, attributed to computer's network, picture archiving and communication system (PACS). is invented for hospitals to promote management efficiency, reduce personnel cost, and be filmless. The study which adopted general diagnostic X-ray machines (named TOSHIBA KXO-50G and TOSHIBA-15R) and a digital image plate of digital radiography system (DRS: typed Canon CXDI-11) before and after PACS installed comprised three groups, each of which had one X-ray technician excluded for comparative analysis of each technician's hourly work loading and patient waiting time. Resultantly, before and after the installment, more than a half of the waiting time was lessened, and the loading was not that much different. After the exclusion, the mean patient amount (1.6) for the loading was added, but the mean waiting time decreased 20-68 seconds. After PACS and DRS applied, the work efficiency was improved and the waiting time was shortened. As for the hospital, the management efficiency progressed, and the workforce cost was reduced. As to the technicians, the designation, rules to take holidays, professional trainings, and image quality control have become stricter. For the hospital, patients, and technicians, a win-win-win situation was achieved.
