隨著神經放射影像學的突飛猛進,許多過去只能靠病理解剖診斷的腦部病變被列入臨床研究,而有進一步的了解。腦部微出血(CMB)是近年來逐漸受到重視的一個課題[1]。本次的研究是收集接受頭部磁振造影健康檢查之受檢者,將常規的序列加上磁敏感加權影像(SWI),利用它對去氧血紅素及其它血液生成物有獨特的磁敏感度,將可以精確顯示腦部微小的出血點、腦瘤和動靜脈畸形異常等,來觀察SWI對於腦部是否提供更精密的評估[2]。腦部微出血的早期診斷和治療一直是臨床關注的問題,成功的關鍵在於早期內發現,而常規的電腦斷層攝影(CT)和磁振造影(MRI)檢查已不能滿足目前臨床的要求。但是常規檢查對腦中最初幾個小時內的缺血性改變或微出血皆不太敏感,僅可能約50%左右的患者可能有異常發現。本研究希望藉由統計結果能夠提供醫師對於此類無明顯症狀的受檢者相關醫療資訊,以作爲預防醫學上的參考及日後的評估。

英文摘要

With the many recent advances in neuroradiology, cerebral disorders that can only be previously diagnosed by histopathology are now possible research topics in the clinical setting. One subject that has gained more clinical attention in recent years is regarding cerebral microbleeds (CMB) [1]. This study collected data a large group of subjects who have undergone health examination at Shuang Ho Hospital and received a magnetic resonance imaging (MRI) scan of the brain. In addition to the routine sequences, a susceptibility-weighted imaging (SWI) sequence was added to detect any magnetic susceptibility artifacts from deoxyhemoglobin and other blood degradation products, and to assess if this sequence can provide more accurate assessment of microbleeds, brain tumors and arteriovenous malformations. The detection and treatment of cerebral microbleeds has been a subject of great interest, but successful treatment hinges on early imaging detection. Computed tomography (CT) and routine MRI sequences are inadequate for this purpose, as they are insensitive to hyperacute ischemia and microbleeds. The data from of this study illustrates the demographics and imaging findings of these asymptomatic subjects with cerebral microbleeds, with the aim at providing a reference data for preventive medicine and future studies.
