背景:以加護病房內之工作環境為研究目標,探討移動式的X光攝影檢查對工作同仁可能之輻射暴露影響。方法:以台灣西部某區域教學醫院之內科加護病房為統計目標,採用定點累計的方法,選定具有代表性的位置作為輻射偵測點,採熱發光劑量佩章為偵測工具,以每一個月為單位,統計一年的期間,讀數分析的內容包含有深部等效劑量以及淺部等效劑量,其結果以Kruskal-Wallis檢定(H test)分析。結果:包含背景值監控在內,於加護病房內、外共設置20個監測點,總計分析240個輻射劑量數值。並依劑量佩章所在位置之性質分做六個分類。結果發現,各主要分類間之結果並無呈現統計之意義(p=0.166)。將背景數值與其餘位置做比較,也並無統計意義出現(淺部等效劑量p=0.423;深部等效劑量p=0.199)。除此之外,各偵測點之月平均值也都在法定限制的安全範圍之內。結論:醫院醫療作業中,輻射作業經常是必要且無法避免的。本研究之結果分析中指出,移動式的X光機攝影檢查其實對人員所造成的輻射劑量是相當有限的。對於工作上無可避免的輻射暴露,應可透過合理的規劃、在職繼續教育的觀念澄清、並以實際的檢測數據說明,減少同仁對此不必要的恐懼心理和擔心情緒的產生。

英文摘要

Background: To explore the status of the environmental radiation exposure of mobile X-ray at the intensive care unit of the hospital.Methods: The fixed point accumulative method by thermoluminescence dosimeter (TLD)
for radiation detection was conducted to an intensive care unit in a regional teaching hospital of western Taiwan. The TLD detector was placed at different specific locations to represent staffs daily working situations. Monthly accumulation was obtained for one year and the
Kruskal-Wallis test (H test) analysis was acquired.Results: Twenty points were set and totally 240 radiation exposure values were acquired
for the monitoring of the radiation exposure during study period. The data were then divided into six categories. The results showed that there was no any significant statistical difference in these categories (p = 0.166). As compared with the background TLD value, it was also no
statistically significant demonstrated (shallow dose equivalent, p = 0.423; deep dose equivalent,p = 0.199). In addition, all TLD data were within the safe range.Conclusion: Radiation monitoring is essential and necessary to be performed in a hospital. In this study, our results illustrate that only limited radiation exposure was detected at an intense care unit environment. Because there are still unavoidable radiation works around the staffs, thoroughly and properly facilities planning, continuing education, and the actual monitor processes would be helpful in reducing the unneeded awareness regarding the safety issues from employees.
