過去許多研究對智能障礙者(以下簡稱智障者)健康特質、照護需求利用等議題進行探討,缺乏針對女性智障者進行研究,容易忽略了性別的個別效應,低估女性智障者的健康照護問題。因此本研究目的為瞭解女性智障者經歷月經之經驗、感受與處理困難等,初探她們此面向的健康需求。本研究為一質性研究,並以深度訪談方式收集資料;採立意選樣方式選取本研究樣本,共計45位,包含輕度或中度女性智障者、個案母親與機構主要照顧者。在資料分析上是以普遍性定義理論、個案摘要與歸納方法等方式來進行分析與整理。本研究結果發現女性智障者本身對於初經來潮的反應與其是否曾在學校接受過相關健康教育有關,在自理能力訓練的成效上與智障者的障礙程度有很大的關係;在健康問題上智障者月經來潮的不適症狀與一般女性無異,以不明腹痛居多,但合併癲癎及精神疾病的智障者在月經來潮時,其發作頻次較高。在月經處理教育訓練上,個案必須以實地演練並反覆教導的訓練方式,才能達到自理訓練的成效。個案對月經來潮的態度偏向負面;但家長的態度較為正向,並且認為自己還有能力照顧時,不會貿然讓智障者接受切除子宮手術,機構老師則表示缺乏適當月經處理或衛生教材是教育訓練上的困境。另外從月經來潮議題所延伸的兩性關係、結紮與子宮切除等,在智能障礙群體裡更受到家長的關注,家屬們期待國家相關單位能夠針對這些道德爭議的議題舉辦各項座談會或提供相關資訊給弱勢群體;另外也發現智能障礙婦女在婦科醫療及預防保健相關服務上,使用率極低;顯示智能障礙婦女的衛生問題將是國內婦女相關疾病預防的死角之一。

英文摘要

People with intellectual disabilities experience communication problem, this limits them in expressing their concern about their own health and may not be able to make choices about their health and their lifestyle. Women with intellectual disabilities appear to be at least as likely to experience menstrual problems as other women. Such problems may be experienced differently and more negatively and may not always be recognized appropriately. Therefore, menstruation appears to be problematic for this group of people. There is few evidence has existed in the literature to describe the menstrual problems which women with intellectual disabilities experienced, particularly in Taiwan. The aim of this study is to explore the experience of women with intellectual disabilities based on perspectives of themselves, their mothers and the institutional caregivers, to exam their role and perception in menstrual problems for them. We recruit three disability welfare institutions which supports full day services for people with intellectual disabilities in Taiwan. The management authorities of institutions have approved this project and involved in this research. The present study employed in-depth qualitative interviews design which recruited 45 participants in the disability institutions. A semi-structured qualitative schedule has developed and used as a guideline to interview the participants. The information of experience of women with intellectual disabilities on issues of personal care, menstrual training and education, the feelings and health needs of menstruation and the opinions/beliefs/preferences/roles of their mothers and caregivers on the menstrual experiences toward women with intellectual disabilities will present in this study. In addition, this study will provide very practical implications for service providers, caregivers and the clients with intellectual disabilities themselves, to incorporate the training, information, resource and communication needs of all concerns of menstruation for women with intellectual disabilities.
