Organ transplantation is sometimes the last resort to save and prolong patients' lives and attempt to improve quality of patient care. However, in Taiwan the numbers of patients who wait for organ transplantation far exceeds the numbers of donors. As a result, many patients died before receiving organ transplantation. The purpose of this study was to determine the willingness of college nursing stl1dcnts to donate organs upon their death. A cross-sectional survey was conducted in a junior nursing college in northern Taiwan. Stratified cluster random sampling was performed using grade as a strata and class as a sampling unit. A well-established self-report questionnaire of organ donation (including willingness, attitude, and know1edge) was used to collect information. In April 1999, 285 valid returned questionnaires were analyzed. Results were as follows. On the question of personal willingness to donate an organ, 22.5% of the individuals responded that they were willing and had signed an organ donation card; 11.6% were willing but had not signed an organ donation card; 57.2% had thought about donating but not decided yet; 6.3% had considered and discussed it with family members; and 2.5% were unwilling The mean score (standard deviation) for three attitude factors were: 3.0 (1.1) for "obstructing organ donation", 2.2 (1.0) for "assure benefits of organ donation", and, 4.2 (1.2) for "uncertain benefits of organ donation". On average, 6.2 out of ten knowledge questions were answered correctly. Statistical analysis revealed that the three attitude factors were important in affecting one's personal willingness about organ donation. For the question "when your family member is dying, will you sign a document to donate your family member's organs?", 12.6% answered "no", 42.1% answered "do not know", and 45.3% answered "yes". The attitudinal factor, "assure benefits of organ donation", showed statistical importance in decision-making.
