目 的:本研究主要在探討12 週的中低強度運動訓練對第二型糖尿病中老年人之生理指標及血液指標之影響。方 法:採立意取樣,在桃園市某照護中心收案,患有糖尿病之中老年人為受試對象,共28 名。給予每週3 次、每次60 分鐘之中低強度(最大心跳率55%-70%)有氧訓練,共12 週。分別在訓練前、後實施生理指標及血液指標檢查,所得資料以paired t-test 分析。研究結果:經過12 週的規律有氧運動後,所有的生理指標及血液指標皆達到統計意義上顯著改善(p < .01)。BMI 由原來的27.5 ± 4.1 kg/m2 下降至26.2 ± 3.9 kg/m2;收縮壓由原來的143 ± 16.2 mmHg 下降至129 ± 14.1 mmHg;舒張壓由原來的79 ± 9.7mmHg 下降至75 ± 10.2 mmHg;體脂肪由原來的35.2 ± 7.6 %下降至30.8 ± 5.5 %;腰圍由原來的87.1 ± 8.1 公分下降至84.6 ± 7.3 公分。空腹血糖由原來的212 ± 24.7mg/dl 下降至142 ± 26.2 mg/dl;糖化血色素(HbA1C)由原來的9.3 ± 0.8 %下降至6.9 ± 0.8 %;總膽固醇由原來的211 ± 26.6 mg/dl 下降至162 ± 24.5 mg/dl;三酸甘油酯由原來的205 ± 92.2 mg/dl 下降至144 ± 49.6 mg/dl。實務應用:本研究結果可提供健康醫療團隊者研擬中老年人相關健康促進運動計畫之參考,期能藉由運動介入有效改善糖尿病患者之生理指標及血液指標,增進其生活獨立性,擁有優質的生活品質。

英文摘要

This study was to explore the effects of an exercise intervention on physical indicators and blood indicators in middle-aged and older people with type 2 DM.Methods:The study used purposive sampling. Twenty-eight voluntary subjects aged 40 years and over were recruited in one long term care center that had regular return to hospital diabetes mellitus
(DM) OPD in Taoyuan City. Low to moderate exercise intervention was undertaken three times per week (60 minutes per session, 55%-70% maximum heart rate) for 12 weeks. Physical indicators and blood indicators were examined before and after the program. Data were analyzed by SPSS 10.0. All of the compared data were examined by paired t-test.Results:Results showed significant improvements in all data (p < .01). BMI was from 26.7 ± 3.9 to 25.3 ± 3.6 kg/m2, systolic pressure was from 143 ± 16.2 mmHg to 129 ± 14.1 mmHg, diastolic pressure was from 79 ± 9.7 mmHg to 75 ± 10.2 mmHg, body fat was from 32.9 ± 5.9 to 29.0 ± 4.2%; waist circumstance was from 86.5 ± 8.5 to 84.3 ± 7.9 cm; hip circumstance was from 97.1 ± 6.4
to 95.3 ± 5.9; Fasting blood sugar was from 209.6 ± 22.8 to 146.5 ± 25.9 mg/dl; HbA1c was from 9.6 ± 0.9 to 7.1 ± 0.8%; triglyceride was from 222.4 ± 105.5 to 152.0 ± 53.4 mg/dl; total cholesterol was from 211.2 ± 28.8 to 161.5 ± 22.1 mg/dl.Implications for practice:Results might serve as useful guidance for community health professionals designing health
promotion plans for middle-aged and older people in the community with type 2 DM. The study prompts the hope that physical indicators and blood indicators can be improved effectively through regular exercise, thereby promoting quality of life in type 2 DM patients.
