本文運用是家庭評估模式照顧一位失能老人及其家屬之經驗，護理期間由98 年7 月21日至7 月30 日。於住院期間，藉由會談和召開家屬會議，並應用Friedman 家庭評估模式為評估工具收集資料整理分析，發現家庭成員間，常以電話聯繫溝通，當面臨決策時，大兒子和小兒子都不敢決定且會出現爭執。住院期間，經評估發現有討論出院後續照顧和經濟費用支出問題，而無法達成共識，有家庭因應能力失調、家庭運作過程改變之問題。住院期間，經由召開家屬會議，討論個案家庭所面臨的問題，協助家庭成員發展相互支持功能，並教導家屬返家照顧技巧及提供社會福利資源，予家屬能有多元化選擇。希望能藉由護理人員的協助和社會資源的介入，讓家庭成員間能運用有效的因應行為，共同解決個案後續照顧問題，以做為類似個案照護之參考。

This text usage family valuation mode looks after a disability elder and its family members of experience, nursing period is from July 21 to July 30, 98 years. In hospitalized period, by talks and convene family members' meeting, and apply the family of Friedman valuation mode to collect data sorting analysis for evaluating tool, find a family member, often with the telephone contact communicate, the big son and children sons all dare not decision and will appear quarrel while directly facing decision. Hospitalization period, discuss hospital discharge the follow-up care and economic overhead problem can't reach a consensus and evaluate there is family in this family respond ability maladjustment and family operation process change of problem. Hospitalization period, through convene family members meeting discuss the problem that the individual case family faces, help a family the member develop mutual support function, and teach the family members how to come home care technique and provide a social welfare resource, can have diversified choice to the family members. Hope by getting involved of the nursing personnel's help and social resource, let family member can make use of to effectively respond behavior and work out the care problem of the individual case follow-up together, with be used as to is similar to
the reference of individual case care.
